1-Year Transitional Housing Program
Coming Fall 2026
Our 1-year housing program is a structured, relationship-based housing program designed to support families who are ready to move forward, providing housing, mentorship, and financial pathways toward long-term independence. The program will span over a year and be divided into four phases.
For the duration of the one-year program, rent will be offered at a subsidized rate to allow the family to financially re-stabilize while they continue their program participation. Participants must go through an intake process and sign an agreement to our housing requirements, which includes a tailored care plan, based on their specific needs.

How do I qualify for transitional housing?
Participant/family lives in Lake County or the Northwest Suburbs of Cook County
Background check completed
Full-time employment required
Transportation plan in place
Willingness to engage in ongoing mentorship (weekly to biweekly)
Demonstrated follow-through and personal responsibility
Willingness to create and follow a budget
Ability to contribute to monthly program costs (scaled over time)
Readiness to actively work toward long-term housing independence
What does the transitional housing program entail?
Up to 12 months of structured housing support
Placement in stable housing at a reduced rate
Weekly to biweekly mentorship, depending on program phase
Personalized goal setting, budgeting, and housing plan development
Built-in accountability around employment, finances, and communication
Support through setbacks, including job loss or financial disruption
Progression based on readiness, not just time in program
