1-Year Transitional Housing Program
Coming Fall 2026
Our 1-year housing program is a structured, relationship-based housing program designed to support families who are ready to move forward, providing housing, mentorship, and financial pathways toward long-term independence. The program will span over a year and be divided into four phases.
The first month is fully funded by the organization to allow for healing and engagement in the program. The subsequent months are subsidized, all while families continue their program participation. Participants must go through an intake process and sign an agreement to our housing requirements, which includes a tailored care plan, based on their specific needs.

How do I qualify for transitional housing?
Participant/family lives in Lake County or the Northwest Suburbs of Cook County
Background check completed
Full-time employment required
Transportation plan in place
Willingness to engage in ongoing mentorship (weekly to biweekly)
Demonstrated follow-through and personal responsibility
Willingness to create and follow a budget
Ability to contribute to monthly program costs (scaled over time)
Readiness to actively work toward long-term housing independence
What does the transitional housing program entail?
Up to 12 months of structured housing support (plus 1-month assessment period)
Initial assessment month to evaluate readiness and program fit
Placement in stable housing with increasing financial responsibility over time
Weekly to biweekly mentorship, depending on phase
Personalized goal setting, budgeting, and housing plan development
Built-in accountability around employment, finances, and communication
Support through setbacks, including job loss or financial disruption
Progression based on readiness, not just time in program
